Practical guide
AI scribe for clinical documentation
An ambient AI scribe listens to the consultation, drafts a structured note from it, and — on some products — writes that note back into the EHR. The listening is the part every vendor has solved. The two that separate them are the write-back and the review step: whether the note reaches the chart without copy-and-paste, and how long it takes you to check a draft you did not write. A scribe that saves four minutes of typing and costs three minutes of proofreading has saved one minute.
§This is general information for practitioners, not medical or legal advice. Recording a patient encounter is governed by state law, your professional obligations and your employer’s policy — see recording people lawfully. There is no HIPAA certification scheme, so no vendor holds one; ask for a signed Business Associate Agreement and written answers on where audio is processed and how long it is retained. Scriben’s clinical console is in preview and not yet available to buy — the pages here rank other vendors, not us. The pen ships today at $129; join the healthcare preview for the rest.
Step by step
How it actually works.
01 / Capture is the solved part
Every ambient scribe on the market transcribes a consultation well enough. Accuracy differences between the major products are real but small, and they are not where your time goes. Do not choose on transcription benchmarks alone.
02 / The draft is a draft
The model writes a structured note from the conversation. It will occasionally attribute a symptom to the wrong speaker, carry a negation the wrong way, or invent a plausible tidy-up. You are signing it, so you are reading it — budget that time honestly when you calculate what the tool saves.
03 / Write-back is where the money is
A note that lands in the chart without copy-and-paste is worth several times one that does not, and it is usually the feature behind the top tier or the enterprise contract. On Freed it is the $119 plan, not the $39 one. Ask which EHRs, and whether it writes structured fields or a blob of text.
04 / Coding is a separate product
ICD-10 and CPT suggestions are billed separately by most vendors and are a different accuracy problem from transcription. Treat any coding claim as something to pilot, not something to buy on.
05 / The conversation outside the room
Every ambient scribe is built for the consultation. The handover in the corridor, the call from the car, the family meeting in a side room — none of them are captured by a tool that needs to be opened and started in front of a patient.
Questions
The ones people actually ask.
What is an AI scribe in healthcare?
Is an AI scribe HIPAA compliant?
How much does an AI medical scribe cost?
Does an AI scribe replace a medical scribe?
Can I use one for telehealth?
Where this comes from
We make a pen that does this.
Scriben is a real ballpoint that records the room to its own storage for up to 18 hours, with no phone and no network, then turns the conversation into a transcript, a summary and the work that follows. It is $129 once. This page would be worth reading whether or not you buy one — that is rather the point of it.
